Seafood Import Monitoring Program Essential Forms and Documents
The Seafood Import Monitoring Program, or SIMP, establishes the reporting and recordkeeping requirements needed to prevent illegal, unreported, and unregulated fishing and/or misrepresented seafood from entering U.S. commerce. SIMP provides updated harmonized tariff schedule codes, three alpha codes, and catch certificate forms.

Model Catch Certificate
The Model Catch Certificate form is used for recording harvest events, both wild-capture and aquaculture. This form consists of the following parts:
- Justification for data elements and format for electronic reporting.
- Certificate for traceability harvest and landing receipt.
- Model reprocessing certificate for traceability primary or secondary processing.
- Model transshipping certificate for traceability.

Model Aggregated Catch Certificate
The model aggregated catch certificate form is used for recording harvest events, both wild-capture and aquaculture. This form consists of two parts—the justification for data elements and format for electronic reporting and the certificate for traceability harvest and landing receipt for small-scale harvest events.
